There will be an optional 5K run/walk at 6pm. Please select if you would like to participate and check the wavier below. Your registration is included in the cost of your apparel. 

Join us at the El Zagal Shrine on Aug 13th from 5-8 for a benefit. There will be a BBQ dinner, bake sale, silent and live auction items, a wine and liquor pull, live music, and a few surprises.  Please come for a night of fun in support of Dave, Diane, and family. 

Waiver and Release of Liability for participating in the 5K run/walk:

I acknowledge and agree to the following terms and conditions:

1. Assumption of Risk: I understand that participating in the 5K Run/Walk (the "Event") involves risks and dangers, including but not limited to falls, contact with other participants, the effects of weather, traffic, and road conditions. I knowingly assume all risks associated with my participation in the Event.

2. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Shirts from Fargo, its officers, directors, employees, agents, volunteers, sponsors, and any other associated parties (the "Released Parties") from any and all claims, demands, actions, or causes of action, of any kind, including but not limited to personal injury, death, or property damage, arising out of or related to my participation in the Event, whether arising from the negligence of the Released Parties or otherwise.

3. Medical Treatment: I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during the Event. I understand that I am responsible for any costs incurred as a result of such medical treatment.

4. Personal Responsibility: I certify that I am physically fit and have trained sufficiently for this Event. I acknowledge that I am solely responsible for my health and safety during the Event and will follow all rules and instructions provided by the Event organizers.

5. Photography and Media Release: I grant permission to Shirts from Fargo to use any photographs, motion pictures, recordings, or any other record of my participation in the Event for any legitimate purpose, including commercial advertising without compensation to me.

6. Legal Age and Competence: I affirm that I am of legal age and fully competent to enter into this agreement, or if I am under 18 years old, my parent or legal guardian has read and agreed to the terms of this waiver on my behalf.

By opting into the event participation, I certify that I have read this waiver and release of liability, fully understand its terms, and agree to it voluntarily without any inducement.
